If you are experiencing an urgent issue related to your medication, or if you are having side effects or concerns about how your medication is working, please contact your provider directly or call our office so we can support you as soon as possible.
To help us process your request efficiently, we ask that you complete all sections of the form as accurately as possible. Be sure to include the name of the medication, dosage, prescribing provider, and your preferred pharmacy information. Refill requests may take up to 2 business days to process, so we recommend submitting your request before you run out of medication.
Keep in mind that refills are provided at the discretion of your prescribing provider and may require a follow-up appointment. In some cases, we may need to review your treatment plan, check in on how the medication is working for you, or ensure that proper monitoring is in place.
